From the final user point of view, the paradigm is simple::

Observation [Context] -> Defect -> Symptoms -> Causes + Solutions [+ Security concerns]

Final user view:
He/she obtains the list of potential defects and associated development modes on the basis of his/her observation of the defect and, optionally a context element... Example.

The observation qualification depends on the domain. Below are two examples where the observation is based on the product structure (because in this case, the process is rapid) and another based on the process (because the production involves several dinstinct proces steps and time is left to observe the intermediate product). Examples :
In both examples, the context is optional, but discriminant when specified. When specified, only the development modes related to (conditionned by) it are displayed.

Just a word, of a part of it can be used to check for a defect name, description or a development mode..

Initialisation additions and updates:
Management pages allows to, add a defect, add one or up to three pictures to illustrate a defect, add a development mode or obtain the integral list of defects, or a part of them for edition.

One for an example of product structure, the other one for process decomposition. In both case, the "view" link leads to the final user view, the "Edit" links leads to the administration pages.

Technical details :
Server: The supporting server tool may be installed in any PC (window$ based or OSX) or any mainframe supporting civilised standards.
It requires a web server (Apache or IIS) ; a simple SQL server (MySQL or M$$QL).
Client: Any civilised web browser, test have been successful with Safari, Internet explorer and Firefox.
